I voluntarily seek services provided by Truthbase Theatricals Inc. and acknowledge that I am potentially increasing my risk of exposure to the Coronavirus/COVID-19 by participating or attending a TbT event. I acknowledge that I must comply with all set procedures to reduce the spread while attending in-person classes or events. All procedures outlined below apply to all individuals regardless of vaccination status. I further acknowledge that Truthbase Theatricals, Inc. (TbT) has put in place reasonable, preventative measures to reduce the spread of the Coronavirus/COVID-19.

Families will be asked to do a health screening at home prior to attending a TbT event or a health screening of their child before sending their child to each class or rehearsal. Individuals who do not pass the screening will not be permitted to attend the event, class or rehearsal.  All event participants/students are welcome to wear a mask, however, it is no longer required.

Families and teachers are asked to self-report if they or a member of their household have symptoms of COVID-19, a positive test for COVID-19 or were exposed to someone with COVID-19 within the past 5 days. 

I attest that the following is true for any person in my family attending an in-person TbT event or class throughout the session.  If any of the following is not true at any point throughout the session, I will notify TbT staff at administrator@tbtnyc.org immediately to report the issue.  TbT will follow CDC guidelines for further procedures in both class and rehearsal on a case-by-case basis.

  • I am not experiencing any symptoms of illness such as a persistent cough, shortness of breath or difficulty breathing, fever (greater than 100 degrees F), muscle pain, headache, sore throat, or new loss of taste or smell.
  • I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19. 
  • I have not tested positive with Coronavirus/COVID-19 or am cleared as non-contagious by state or local public health authorities.
  • I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

I further acknowledge that Truthbase Theatricals Inc. cannot guarantee that my student, myself or any other family members will not become infected with the Coronavirus/COVID-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, TbT staff/teachers, and other TbT students and their families.

I hereby release and agree to hold Truthbase Theatricals Inc. harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act by the organization, or that may otherwise arise in any way in connection with any services received from TbT. I understand that this release discharges the organization from any liability or claim that I, my heirs, or any personal representatives may have against TbT with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from TbT. This liability waiver and release extends to the organization together with all owners, partners, and employees.

Truthbase Theatricals Inc. may make changes of any nature at any time and from time to time to its classes, calendar, schedule, standards, policies, requirements, rules, and procedures whenever TbT deems it necessary or desirable in connection with the on-going COVID-19 pandemic. This may include, but is not limited to, changes in modes or methods of delivery of classes, modifying or rescheduling classes, and requiring compliance with new, additional, or modified standards, policies, procedures, or rules intended for the protection of the health and safety of our community.
